Use this form so we have all your contact information and can get you a USA Wrestling Membership. The membership is important for camps and tournaments and can provide supplemental insurance. One of our wrestlers was reimbursed $500 after an injury.

All fields are required except Parent/Guardian 2 information and the USA Wrestling membership number.
